What to Bring to Your Orthopaedic Appointment in Melbourne

To ensure a thorough and efficient consultation, please bring the following to your appointment with your orthopaedic surgeon:

• Referral letter from your GP or specialist

• List of current medications

• Private health insurance details

• DVA card, Healthcare Card, Pension Card, WorkCover or TAC claim number (if applicable)

• Relevant imaging including X-rays, CT scan

• s, MRI scans, or ultrasound reports

• Radiologist reports for all imaging

• Relevant blood test results

• Correspondence from other healthcare professionals

If you do not have copies of your imaging, please contact our rooms prior to your appointment so we can arrange electronic access.
